Bloom Within Counseling, PLLC is committed to ethical practice, client rights, and full compliance with applicable Texas law. The information below outlines your rights as a client and how to access important resources.

Client Records Request

Texas House Bill 4224

In accordance with Texas House Bill 4224 and applicable Texas law, Bloom Within Counseling, PLLC is committed to transparency and timely access to client records.

Clients may request copies of their counseling records by submitting a written request via email to melanieburns@bloom-within-counseling.com.

Please include the following in your request:

  • Client's full name
  • Date of birth
  • Type of records requested
  • Preferred method of delivery (secure electronic or paper, if applicable)

Records will be provided within the timeframe required by Texas law — generally within 15 business days of receiving a valid written request, unless an extension is permitted by law. Reasonable fees may apply as allowed by statute.

Texas State Board of Examiners of Professional Counselors

Clients may verify licensure status or file a complaint with the Texas licensing board at bhec.texas.gov. Select "Professional Counselors" for licensing and complaint information.

The Texas State Board of Examiners of Professional Counselors regulates the practice of Licensed Professional Counselors (LPCs) in Texas.
